Welcome to our membership page.  Below you will find our Rules and Regulations and our Agency Application form.  Please read our Rules and Regulations as your membership is reliant upon your organisation representative complying with them.

You will need to complete the Agency Registration if applying for membership or renewing membership.

All applications for Membership or Renewal of Membership must contain the following:

1.    Competed Food Relief NQ Agency Application form.

2.    Current Agency Information Sheet.

3.   Proof that the agency has not-for-profit organisation status or is a public benevolent institution or the agency's certificate of tax exemption on the basis as a welfare organisation.

4.    Proof that the agency is covered by current public and products liability insurance.

5.    Membership fees for Jan 2012 to Dec 2014 is $40.00 (forty dollars).  This amount must be paid prior to acceptance of membership.
